These health policies are that of Baltimore County Public Schools and the Board of Education of Baltimore County, Maryland. Discretionary Medication Baltimore County Public Schools and the Baltimore County Department of Health has developed the use of discretionary medications (over-the-counter) for various symptom relief. The Director of the Health Department and the Coordinator of Health Services developed the school nurse protocols to help students who are experiencing symptoms so that they can remain in class feeling better and learning. Parent permission for this program is obtain each year. A letter explaining this program in sent with the permission form. Emergency Contact Information / Annual Health Update It is important that the school nurse be able to reach you in the event of an emergency. Updated information is obtained at the beginning of each school year. If the information changes during the course of the year, you may print the form from this site , complete it, and send it to school with your child. Please click here to get the form.
Medication Policy Click here to go to the Medication Policy. Release Of Records Before the school can send any information to an outside person or agency we must have your written permission. For students who take medication at school, teacher reports can be sent to your health care provider so that the medication effectiveness can be assessed . Click here to download a copy of the form. After it is completed, please send it to the school nurse with your child.
Physical Education Excuses Parents can write a note to the physical education teacher for up to a week. If a child needs to miss more than one week of physical education class, a doctors note is needed that includes the reason for needing to be excluded and a projected date when the child can resume Physical Education. |
